Upon submission of this form you confirm you:

Understand that if i choose to relocate Positive Solutions Ltd Analyst System between sites i am liable for any damage/break-down caused to the system.

Understand that if a Positive Solutions Engineer is required to attend site to resolve hardware and/or software problems caused by the relocation, the following will apply:

• You will be invoiced for a minimum of half a day call-out charge.
• Full day call out charge will be applied if the Engineer is on site for over 4 hours.
• The engineers travelling time is included and classes as total call-out time.
• The call-out rate does not include any parts fitted to repair or replace equipment where necessary.
• An accurate evaluation of the parts and costs required to repair any equipment can only be identified during an on-site inspection. Following this inspection, if the cost is proved to be unacceptable or the unit is beyond economical repair, the relevant call out charge would still apply.
• Prices are subject to change, however we will honour this agreement if the form is submitted within 21 days of the date reported.
• IMPORTANT: Each relocated terminal must be sited within 2 meters of a network and power point.

Engineer Call-Out Charges*

Half Day (Up to 4 hours on site) £295.00 (ex VAT)

Full Day (Over 4 hours on site) £500.00 (ex VAT)
